Order Forms

Choose the form that matches your shirt design. Start with its instructions, then complete one order form—Excel or PDF. If you can use Excel, we recommend it for its built-in guidance and automated features.

Single-color shirts

For designs printed in one ink color. Read the guide first, then choose Excel or PDF.

1. Read the PDF instructions first

2. Choose ONE order form: Excel OR PDF

Use either format—not both. Excel is recommended for its built-in guidance; choose PDF to print and complete by hand.

Before you mail your order

Check that your drawings, sizes, wording, payment information and contact details are complete. Follow the drawing and mailing directions, and review shipping and turnaround information before planning your date. We recommend UPS or FedEx with tracking because postal delays may affect incoming orders.
